What's Covered?
Insurances have many different plans and benefits. We recommend that you contact your insurance company for information regarding co-pays, co-insurance and deductibles for "Out Patient Physical Therapy" services. You can find a Member Services number on your insurance card and they will be able to assist you. For those using Workman's Compensation benefits, please call your employer's Human Resources or Personnel Dept for information.
